Family Involvement Plan

housing transition cover

Helping family members and friends determine the ways they are able to offer support and assistance in the care of a loved one

Benefits

  • You will have a communication plan that everyone can agree on
  • You will know each person’s strengths and skills, and who to call on for certain tasks or responsibilities
  • No matter where they live, other family members can be included in the process
  • Stress is reduced by knowing who is responsible for what

What this would cost you elsewhere?

Toghers currently knows of no other company or individual offering this type of plan.

Family Involvement Plan: $100

A plan to assist and coordinate participation of family members and friends in the care of the care recipient.

Whether you are caring for a family member or a friend, chances are you are not the only individual able and willing to provide some type of care for the care recipient.

  • Do you know who is willing to step in and help?
  • Do you know who the care recipient wants help from, or what they want to happen?
  • Have you thought about what others can do to support the effort?
  • Do you want to reduce discord and improve communication?

Giving everyone an opportunity to choose their role based on their individual strengths and boundaries will provide the most stress-free environment for the care recipient and for each of the caregivers.

What is involved?

You will be guided through a series of questions that will help the Caregiver Coach® determine which options make the most sense in your situation. The Coach will offer tips along the way that help you understand why one choice might be better than another. You will receive tools you can distribute to family members and friends to narrow down what they can do to help. Once you approve the recommendations the Coach makes in a Plan Overview, you can go through each task at your own pace. After you have made each decision, the Coach will put that information into a Final Plan that will be sent to you. You can implement the plan at your own pace.
